It is one of the most common questions we hear from prospective customers: “Why do I need to get my equipment inspected?” The short answer is that inspection is not optional — it is required by federal law, reinforced by state regulations, and expected by every major accreditation body. Just as importantly, it protects your patients and your facility. Here is what the rules actually say, and what a proper inspection involves.
The Short Answer: Three Overlapping Requirements
For healthcare facilities in the United States, the obligation to inspect and maintain medical equipment comes from three sources that reinforce one another:
- Federal law (CMS). Medicare/Medicaid-certified facilities must keep all equipment in safe operating condition.
- Electrical safety codes (NFPA 99). Patient care-related electrical equipment must be tested for electrical safety on a defined schedule.
- Accreditation standards. The Joint Commission, DNV, AAAHC, and CARF each require a documented equipment management program.
1. Federal Law — CMS “Safe Operating Condition” (F908)
Every facility that participates in Medicare or Medicaid must comply with the CMS physical-environment requirements at 42 CFR 483.90. For skilled nursing facilities this is enforced through F-tag F908, “Essential Equipment, Safe Operating Condition,” which states:
The facility must maintain all mechanical, electrical and patient care equipment in safe operating condition.
In practice, surveyors expect equipment to be maintained per the manufacturer's recommendations, with documented preventative maintenance and calibration records available on request. This is not limited to a size threshold and there is no waiver — it applies to every certified facility. Hospitals and other provider types carry parallel obligations under the CMS Conditions of Participation.
Why it has teeth: equipment deficiencies under F908 have been cited at the Immediate Jeopardy level — the most serious severity tier — which can trigger civil monetary penalties, a directed plan of correction, and, in extreme cases, loss of Medicare/Medicaid certification.
2. Electrical Safety — NFPA 99 and Survey Tag K0921
The most common inspection driver for electrically powered devices is electrical safety testing under the NFPA 99 Health Care Facilities Code (2012 edition), which CMS has adopted. NFPA 99 Chapter 10 governs Patient Care-Related Electrical Equipment (PCREE) — essentially any electrical device used in patient care — and its requirements are verified through CMS Life Safety Code survey tag K0921.
A PCREE electrical safety inspection measures four things against defined acceptance limits:

Acceptance limits are drawn from NFPA 99 (2012) §10.3. Exact limits depend on device type and grounding configuration; a facility's policy and the manufacturer's specifications govern.
How often is testing required?
A frequent point of confusion: NFPA 99 does not mandate a single fixed interval. The facility determines its own testing frequency, provided it is reasonable and aligned with manufacturer guidance. Because most manufacturers recommend annual electrical safety testing, annual inspection has become the industry standard.

3. Accreditation Standards
If your facility is accredited, your accreditor adds its own equipment-management requirements on top of the federal baseline. These bodies generally defer to NFPA 99 and manufacturer recommendations for intervals, but they require a documented program with complete records:
- The Joint Commission (TJC) — EC.02.04.01 and EC.02.04.03 require a complete equipment inventory, a written maintenance plan, and a high completion rate for scheduled maintenance.
- DNV Healthcare — PE.1 through PE.5 physical environment requirements.
- AAAHC (ambulatory / urgent care) — written policies for each device, with on-site surveys every three years.
- CARF (rehabilitation / physical therapy) — documented maintenance, calibration records, and safety inspections.
What's at Stake If You Don't Inspect
Skipping inspections is a compounding risk. A surveyor who finds undocumented or out-of-tolerance equipment can issue a citation that requires a plan of correction and a follow-up survey. Serious findings carry civil monetary penalties. Beyond the regulatory exposure:
- Patient safety. Ground faults and excessive leakage current are exactly the hazards electrical safety testing is designed to catch.
- Liability. An incident involving equipment with no maintenance record is far harder to defend.
- Warranty. Failing to maintain a device per its instructions for use can void the manufacturer's warranty.

Electrical safety and calibration testing requires specialized, regularly calibrated instruments — electrical safety analyzers, pressure and temperature standards, output meters — and a trained biomedical equipment technician (BMET) to run them and interpret the results. The national BMET workforce is in chronic short supply, and most facilities do not have enough equipment to justify a full-time in-house biomed department.
